If we make changes to this notice we will notify you by updating it on our website. Jersey Anaesthetic Group (J.A.G.) Limited will be what is known as the ‘Data Controller’ of the personal data you provide to us.  By Data Controller, this means Jersey Anaesthetic Group (J.A.G.) Limited determines the purposes and way in which any personal data are, or will be, processed. 

In our database we will keep some personal information including:

Your full name, address, date of birth, contact numbers, e-mail address, general practitioner provider details, medical information to/from health professionals regarding the provision of your medical treatment.  

We are permitted by law to collect, use and share your medical information with other medical agencies/health professionals regarding the provision of your medical treatment.  We will not sell, distribute or otherwise disclose your information without your consent, unless required or permitted to do so by law.    

J.A.G. Limited also works with suppliers who may make use of cloud and/or hosted technologies. J.A.G. Limited undertake data security due diligence on our suppliers and ensure that these suppliers conform to appropriate accreditations.
